HEALTH NAVIGATORS
Community Health Navigators (CHNs) provide a wide range of supportive services as part of the California Advancing and Innovating Medi-Cal (CalAIM) initiative. Services provided by CHNs include preventive health services, health education, assistance with navigating the healthcare system, connecting individuals to community resources, and much more.
COMMUNITY HEALTH NAVIGATORS
- CHNs come from diverse backgrounds and have lived experience that aligns with and provides a connection to the community or population being served.
- They offer a range of services including preventive health services, screening and assessment, individual support or advocacy, and health navigation.
- CHNs provide evidence-based, trauma-informed, and culturally responsive preventive services aimed at reducing the incidence of domestic violence, trauma, and related harms.
SERVICES PROVIDED
- Organizing transportation for doctor's appointments.
- Ensuring safe transitions between care settings.
- Assisting with applications for food and housing benefits.
- Offering follow-up care after hospital discharge.
- Facilitating connections with doctors and scheduling appointments.
- Linking individuals to community programs and social services.
- Aiding in understanding prescription medications.
- Helping create healthcare plans tailored to individual needs.
It's important to note that while CHNs offer a wide range of services, there are certain services they do not provide, such as clinical case management requiring a license, childcare, and delivery of medication or medical supplies.
ELIGIBILITY
CHNs work specifically with BlueShield, Molina, and Community Health Group health plans. If you possess medical coverage through any of these health plans you may be eligible.
